Dear Patient and Friend:

As you may already know, Dr. Peter N. Reusswig recently passed away. His sudden illness and passing was a shock to all of us who respected and relied upon him. It is with deep sadness that we inform you that Colorado Pain Management and Anesthesia Consultants, P.C. (“Colorado Pain Management”) closed its practice on Friday, April 27, 2018.

In order to ensure the continued availability of your medical records and to help facilitate your continued medical care, we have contracted with U.S. Anesthesia Partners of Colorado, Inc. (“USAP”) to become the custodian of medical records for Colorado Pain Management. Your original medical records will be maintained by USAP and will be available to you as described below.

We can recommend that USAP and its professionals are dedicated to the provision of excellence in anesthesia and pain management care. If you so choose to have your care managed by USAP, through its staff of experienced physicians, nurses and other support personnel, USAP is more than capable and willing to carry on your treatment at either of its Denver area clinics, located at:

6950 East Belleview Avenue 11960 Lioness Way
Suite 300 Suite 130
Greenwood Village, CO 80111 Parker, CO 80134

If you would like one of the USAP physicians to assume your care you simply need to call (303) 750-8100 to reach USAP’s new patient scheduling partner, Pain Doctor.

As always, you have a right to a copy of your own records. To receive a copy, at no cost to you, please call the number above, and identify yourself as a patient of Colorado Pain Management who is requesting a copy of your medical records.

We will gladly answer any questions and assist you in completing the enclosed authorization CLICK HERE and/or request for a copy of your medical records.

It is always your choice where you wish to receive continued care, and our main goal is to ensure a seamless transition of your medical care. Please CLICK HERE for a list of area pain management doctors. If you decide to continue your care with a medical provider other than USAP, our Pain Doctor operators will be able to send your medical records directly to you or transfer them to another provider of your choosing, at no cost to you.

It is important that, if you have a medical emergency prior to finding another physician, please call “911” or go to your nearest emergency room.

We appreciate your understanding during this difficult time.

Sincerely,

Colorado Pain Management
